FEIA (Fluorescence Enzyme Immunoassay) – quantitative determination of specific IgE antibodies to penicillin.

Description

Penicillin G (benzylpenicillin) is a naturally derived antibiotic produced by industrial fermentation of Penicillium chrysogenum. Small penicillin molecules alone do not induce an immune response; however, the beta-lactam ring of penicillin forms covalent bonds with proteins, creating hapten-protein conjugates. These conjugates trigger allergic reactions, and Penicilloyl G (c1) is the major allergenic determinant of penicillin G with potential cross-reactivity with other penicillins.

Indications

Diagnosis of allergic reactions to penicillin.
Assessment of sensitization level and risk of allergy development.
Monitoring therapy effectiveness.
Evaluation of decreased sensitization after allergen avoidance.
Assessment of immune system status.

Preparation

Blood should be drawn in the morning on an empty stomach or 4 hours after eating.
Drinking plain still water is allowed.
Do not smoke 30 minutes before the test.
For children under 1 year – no food 30-40 minutes before the test; for children 1-5 years – 2-3 hours fasting.

Clinical Significance

Penicillin allergy can manifest as immediate reactions (urticaria, angioedema, bronchospasm, anaphylaxis) and delayed reactions (maculopapular rash, delayed urticaria). Immediate reactions are associated with IgE-mediated mechanisms, while delayed reactions involve T-cell sensitization. The Penicilloyl G (c1) IgE test has high clinical specificity but limited sensitivity, so a negative result does not exclude penicillin allergy. Additional skin tests are recommended for comprehensive diagnosis.
