Ananas comosus, widely known as pineapple, belongs to the Bromeliaceae family and is the second most important tropical fruit worldwide. Its fruits are consumed fresh, cooked, or as juice. Pineapple consumption can cause allergic reactions, including oral allergy syndrome (OAS).

  • Diagnosis of allergic reactions to pineapple.
  • Assessment of sensitization level and risk of allergy development.

  • Blood should be drawn in the morning on an empty stomach or 4 hours after eating.
  • Drinking plain still water is allowed.
  • No smoking for 30 minutes before the test.
  • For children under 1 year – no food 30-40 minutes before the test; for children 1-5 years – 2-3 hours fasting.

Pineapple allergy may be associated with pollen allergy due to the pan-allergen profilin (Ana c1). Bromelain (Ana c2) can cause systemic IgE-mediated reactions, including anaphylaxis. Occupational exposure to bromelain may lead to sensitization and exacerbate allergic reactions. Oral mucosa irritation is often related to pineapple acidity and bromelain enzymatic activity, which is not an IgE-mediated allergy but can complicate diagnosis due to symptom similarity with OAS.
